Practical questions

When HbA1c may be misleading: anaemia and haemoglobin

Sometimes HbA1c does not fit the glucose readings or clinical picture. That is a question for the responsible clinician, not proof that one report is fraudulent. Conditions affecting red blood cells and some haemoglobin variants can change how HbA1c should be used or interpreted.

Bring the history that may change interpretation

Tell the clinician about known anaemia, a haemoglobin disorder, recent transfusion or significant blood loss, and other relevant conditions. Keep the timing with the report. Do not assume that every anaemia affects HbA1c in the same direction or that a personal online correction formula is reliable.

Take discordant reports together

Bring laboratory glucose results, the HbA1c report and a record of home readings if you already monitor them. Ask whether the assay or another diagnostic approach is appropriate. A meter reading and HbA1c cover different time periods; disagreement should be assessed rather than resolved by repeatedly purchasing the same package.

Keep treatment changes clinician-led

Do not increase or stop diabetes treatment because the number seems inconsistent. Ask who will coordinate any alternative testing and explain its limitations. New symptoms of significant illness need timely care; this article does not establish that an HbA1c result is inaccurate in your particular case.

Common questions

Does all anaemia change HbA1c the same way?

No. The condition and test method need professional interpretation.

Can a normal HbA1c settle every glucose concern?

No. The clinical context and test limitations matter.

Should I change medicine from a conflicting report?

Discuss the discrepancy with the treating clinician first.
